Care Coordination & Ongoing Support
Navigating the health care system can be challenging. Our team partners with you to keep the lines of communication open with your primary care physicians, specialists and community services. We are available to attend doctor’s appointments to help ask questions, understand treatment options and answer questions on medical history or needs.
For family members who don’t live nearby, our service provides peace of mind that any changes in their aging loved one will be communicated and updates provided.
At ElderCARE, we recognize the toll that caregiving can take on an individual or family. In the U.S. today, nearly 1 in 5 people are providing care to an adult, and 23% say caregiving has made their own health worse (AARP). In addition, increasingly throughout the pandemic, social isolation has become a significant issue as the aging population has quarantined to be safe. Supporting caregivers and partnering with them is a critical component of our assessment and care plan and recommendations.
